Selective mutism (SM) is a complex anxiety disorder characterized by a consistent inability to speak in specific social situations where there is an expectation for speaking (e.g., at school), despite being able to speak freely and communicate effectively in other settings where the individual feels comfortable and secure. It is more than just shyness or timidity; individuals with selective mutism are literally unable to speak due to an overwhelming 'freeze' response triggered by anxiety and panic in certain situations. This condition usually starts in early childhood, often between ages 2 and 4, and if left untreated, can persist into adolescence and adulthood, potentially leading to isolation, low self-esteem, and social anxiety disorder.

Ayurvedic perspective

Selective Mutism, a modern psychological condition, is not directly named in classical Ayurvedic texts. However, its symptoms, such as anxiety, fear, speech difficulties, and social withdrawal, are addressed within Ayurvedic principles. Ayurveda views such conditions as an imbalance in the doshas, particularly Vata, which governs the nervous system and communication. Emotional factors like anxiety, fear, and stress can manifest as speech problems. Social anxiety, a core component of Selective Mutism, is considered a 'Shareerika - Manasika Vikara' (psychosomatic disorder). Speech delays and difficulties are often attributed to Vata imbalance, weak Agni (digestive fire), and insufficient nourishment of neurological tissues. The proper functioning of the mind (Manas) is considered essential for clear speech. Conditions with similar features, like Autism, are correlated with 'Unmada' (psychiatric disorders) in Ayurvedic literature.
