Autosomal recessive polycystic kidney disease (ARPKD) is a rare genetic disorder that primarily affects the kidneys and liver, often presenting in utero or during infancy. It is characterized by the growth of numerous fluid-filled cysts in the kidneys, which can lead to their enlargement and impaired function. The condition can also cause congenital hepatic fibrosis and other liver problems. ARPKD can lead to serious complications, including end-stage renal failure, and in severe cases, can be life-threatening, particularly in newborns due to underdeveloped lungs.

Ayurvedic perspective

In Ayurveda, Polycystic Kidney Disease (PKD), including Autosomal Recessive Polycystic Kidney Disease (ARPKD), is not described by a direct modern name. Instead, it is understood as an imbalance primarily affecting the 'Mutravaha Srotas' (channels responsible for carrying urine and maintaining fluid balance). Ayurvedic texts correlate renal cysts with 'Granthi' (cysts or nodular growths) where Vata-Kapha dominant Tridosha is involved. The disease is generally linked to an imbalance of Kapha and Vata doshas, with Pitta also playing a role. Vata imbalance can contribute to the formation and growth of cysts, creating irregular movement and dysfunction, while Kapha imbalance may exacerbate fluid retention, swelling, and blockages in the urinary tract due to excessive accumulation. Pitta imbalance can lead to inflammation and damage, potentially causing 'Rakta Dushti' (vitiation of blood). The accumulation of 'Ama' (toxins) due to weak digestive fire ('Agni') is also considered a contributing factor to impaired tissue nutrition and kidney dysfunction. Ayurvedic management focuses on a holistic approach to support kidney function, slow disease progression, and manage symptoms, rather than claiming a complete cure for this genetic condition. The strategies involve balancing the doshas, improving digestion, eliminating toxins, rejuvenating kidney tissue, and enhancing overall vitality. Rasayana (rejuvenating) drugs are used to prevent and repair tissue damage, while Lekhana (scraping) drugs help remove blockages in the channels.
