Von Willebrand disease (Raktapitta)
Von Willebrand disease (VWD) is the most common inherited bleeding disorder, affecting up to 1% of the U.S. population. It is a lifelong condition where the blood does not clot properly due to a deficiency or dysfunction of von Willebrand factor (VWF), a protein essential for blood clotting. VWF helps platelets stick together and to damaged blood vessel walls, and also carries factor VIII, another clotting protein. There are three main types: Type 1 (most common, mild to no symptoms), Type 2 (VWF doesn't work as it should, mild to moderate bleeding), and Type 3 (rarest and most severe, very low or no VWF, serious bleeding issues). Rarely, VWD can be acquired later in life due to other medical conditions or certain medications.
Common symptoms
Bleeding from a cut or injury that lasts longer than 10 minutes.
Frequent nosebleeds (more than 5 a year) that last longer than 10 minutes.
Easy bruising or bruises that are large, raised, or appear for no reason.
Heavy menstrual bleeding (menorrhagia), including soaking through a pad or tampon every 1-2 hours, periods lasting longer than 7 days, or passing blood clots larger than 1 inch.
Heavy bleeding after childbirth or miscarriage.
Heavy bleeding after surgery, including dental work.
Bleeding gums.
Blood in poop or pee.
Sudden bleeding into joints, causing pain, swelling, and stiffness (more common in severe forms like Type 3).
Symptoms of anemia, such as tiredness or shortness of breath, due to heavy bleeding.
Possible causes
Von Willebrand disease is almost always an inherited genetic disorder, passed down from a parent to a child.
It occurs when there is an abnormality in the gene responsible for producing von Willebrand factor (VWF).
This genetic change leads to either low levels of VWF, or VWF that doesn't function correctly.
Most cases of Type 1 and Type 2 VWD are inherited in an autosomal dominant pattern, meaning a child needs to inherit a faulty gene from only one parent.
Type 3 VWD, the most severe form, typically occurs when a child inherits a faulty gene from both parents (autosomal recessive inheritance).
Rarely, VWD can be acquired later in life (acquired von Willebrand syndrome) as a complication of certain cancers, autoimmune disorders (like lupus), heart disease, or from taking certain medicines.
Ayurvedic perspective
In Ayurveda, Von Willebrand disease, characterized by various bleeding tendencies, is primarily understood under the umbrella of 'Raktapitta,' a classical disease entity marked by spontaneous hemorrhages from different bodily orifices due to aggravated Pitta vitiating Rakta Dhatu (blood tissue). Pitta, responsible for heat and transformation, when aggravated, can lead to increased heat and pressure within the body, damaging the walls of blood vessels and causing blood to flow out of various openings. This condition is considered grave and can have systemic implications.
Vata dosha also plays a significant role, particularly in regulating blood flow. An imbalance in Vata can lead to irregular blood flow, dryness, and can even increase the quantity of blood, contributing to excessive bleeding. In some cases, Kapha dosha involvement can manifest as sluggish, pale, or pinkish bleeding with heaviness, or contribute to clotting issues.
Heavy menstrual bleeding, a common symptom of Von Willebrand disease, is specifically referred to as 'Rakta Pradara' or 'Asrigdara' in Ayurveda. This condition is also linked to imbalances in Pitta and Vata doshas, affecting blood circulation and reproductive health, and can be exacerbated by factors like excessive heat, stress, and poor nutrition. Modern interpretations of Raktapitta link it to vascular fragility, platelet dysfunction, and clotting factor deficiencies, aligning with the pathophysiology of Von Willebrand disease.

Vasa (Adhatoda vasica): Extensively used in almost all types of bleeding disorders due to its bitter and astringent taste.
Durva (Cynodon dactylon): Used in home remedies for reducing hematuria and other bleeding.
Lajjalu (Mimosa pudica): Used to cure Raktapradara (excessive menstrual bleeding).
