Heat rash, also known as prickly heat or miliaria, is a common skin condition that occurs when sweat gets trapped under the skin due to blocked sweat ducts. It is particularly prevalent in hot and humid climates and can affect both adults and babies. From an Ayurvedic perspective, heat rash, referred to as Romantika, is viewed as a vitiation of Pitta and Kapha doshas in the sweat channels (svedavaha srotas), leading to ama accumulation and irritation. While generally harmless, it can be itchy and uncomfortable.

In Ayurveda, prickly heat, also known as heat rash or miliaria, is primarily understood as a manifestation of aggravated Pitta dosha, often with involvement of Kapha dosha. Pitta, which governs heat and metabolism, becomes elevated due to factors like excessive sun exposure, spicy foods, or stress, leading to increased internal body heat. This excess heat irritates the skin and causes blockages in the sweat channels (svedavaha srotas). When sweat is trapped beneath the skin, 'ama' (toxins) accumulate, leading to inflammation, redness (erythema), tiny blisters (vesicles), and the characteristic stinging or prickling sensation. While Pitta is the primary dosha involved, Kapha dosha can also contribute by causing blockages and fluid retention in the sweat ducts, leading to more oozing or sticky eruptions. A weakened or erratic digestive fire (agni) can also lead to excess ama formation, which then travels through the bloodstream and aggravates the skin. Individuals with a Pitta-predominant constitution are more prone to prickly heat due to their naturally warmer bodies and tendency to sweat more. Vata dosha imbalances can also contribute to the sharp itching sensation when channels constrict.
