Acute cholecystitis is the sudden inflammation of the gallbladder, a small organ located beneath the liver that stores bile. It commonly occurs when a gallstone blocks the cystic duct, the main opening of the gallbladder, leading to bile buildup, irritation, pressure, and potential infection. This condition is potentially serious due to the risk of complications if not treated promptly.
Common symptoms
Sudden, sharp, or intense pain in the upper right-hand side of the abdomen, which may spread to the right shoulder or back.
Pain that is persistent and does not go away within a few hours, often worsening with deep breaths.
Tenderness over the belly area when touched, often with involuntary guarding of upper abdominal muscles on the right side.
High temperature or fever.
Feeling sick (nausea).
Being sick (vomiting).
Sweating.
Loss of appetite.
Yellowing of the skin and the whites of the eyes (jaundice), which affects up to 1 in 10 people with cholecystitis.
A bulge in the tummy.
Abdominal distension (bloating).
Possible causes
Calculous cholecystitis (most common, ~95% of cases): Occurs when a gallstone or biliary sludge blocks the cystic duct, causing bile to build up and inflame the gallbladder.
Acalculous cholecystitis (less common, but often more serious): Gallbladder inflammation without gallstones. The exact cause is unknown, but it's typically a complication of serious illness, infection, or injury, such as major surgery, severe injuries or burns, sepsis, severe malnutrition, dehydration, or conditions like HIV/AIDS and diabetes.
Bacterial infection in the bile duct system.
Tumors of the pancreas or liver that obstruct bile drainage.
Reduced blood supply (ischemia) to the gallbladder, potentially due to critical illness, trauma, vascular occlusion, or vasculitis.
Gallbladder sludge: A thick material that builds up in the gallbladder.
Ayurvedic perspective
In Ayurveda, acute cholecystitis, often triggered by gallstones, is understood as an imbalance primarily involving Pitta and Kapha doshas, along with impaired Agni (digestive fire) and the accumulation of Ama (toxins). Pitta, which governs digestion and metabolism, becomes aggravated, leading to increased and sharp bile production. Concurrently, Kapha dosha contributes to the stagnation and thickening of bile. The dry quality of Vata dosha can further harden this viscous bile, leading to the formation of stones (Pittashmari) and obstruction in the Pittavaha Srotas (biliary channels). This obstruction and inflammation manifest as gallbladder pain and other symptoms associated with acute cholecystitis.

Kutki (Picrorhiza kurroa): Liver tonic, detoxifies, supports gallbladder health, stimulates bile flow, and reduces inflammation.
Triphala (Amla, Haritaki, Bibhitaki): Gentle detoxifier, promotes regular bowel movements, reduces toxin buildup, and prevents stone formation.
Punarnava (Boerhavia diffusa): Reduces inflammation, improves bile flow, acts as a diuretic, and helps in stone dissolution.
