Actinic keratoses (solar keratoses) (Charma Kushtha / Surya-Atapa Janya Twak Vikara)

Actinic keratoses (AKs) are common, rough, scaly skin patches caused by years of cumulative ultraviolet (UV) radiation exposure. Also known as solar keratoses, they are considered precancerous lesions; if left untreated, they have a 5% to 10% risk of progressing into squamous cell carcinoma (SCC), a type of invasive skin cancer. These growths primarily appear on sun-exposed areas such as the face, scalp, ears, lips, and the backs of hands. Management focuses on local destruction of visible lesions and 'field therapy' to treat surrounding subclinical damage.

In Ayurveda, Actinic Keratoses are interpreted as a chronic condition of the Twacha (skin) falling under the classification of Kushtha (skin diseases), specifically correlating with Charma Kushtha (rough skin thickening) or Surya-Atapa Janya Twak Vikara (sun-exposure induced skin disorder). The condition is a manifestation of prolonged Atapa-Sevana (sun exposure) which causes Agantuja (external) trauma leading to the vitiation of Pitta and Rakta (blood). This thermal stress burns the deeper skin layers (Lohita and Tamra), while the subsequent involvement of Vata leads to the characteristic Parushya (roughness), Khara-sparsha (scaly texture), and Vaivarnya (discoloration). If untreated, the accumulation of Ama (toxins) and persistent Pitta-Rakta vitiation may progress toward Arbuda (malignant growths).
